How fast does Mounjaro work and how much weight do you lose?
You have given yourself your first injection and want to know when you will notice something. The short answer: Mounjaro is active in your body within a day, most people notice less appetite in the first week, and you usually see a difference on the scales after 4 to 8 weeks. The full effect builds up over months, because the dosage goes up step by step.

How fast does Mounjaro work after the injection? The short answer
Mounjaro starts working within a few hours of the injection and reaches its highest concentration in your blood after 8 to 72 hours. You often notice less appetite in the first week. Measurable weight loss usually follows after 4 to 8 weeks. The maximum effect comes at a higher dosage, after a few months.
That difference between "already working" and "already visible" is the core of the answer. Tirzepatide is a GLP-1 and GIP receptor agonist: it suppresses appetite, slows gastric emptying and influences your blood sugar. These effects are there from the first injection, but at the starting dose of 2.5 mg they are deliberately mild. What do you notice in the first week?
In the first week, most people notice three things: feeling full sooner during meals, less appetite between meals and sometimes mild nausea. Your weight usually changes little in that week. Some people notice nothing in the first week; that says nothing about the effect in the longer term.
What you do not need to expect in week 1: a large drop in weight. The starting dose of 2.5 mg is intended to let your body get used to the medication, not to lose as much weight as possible. A drop of 1 or 2 kilos in the first weeks is often mainly fluid and less stomach content.

The timeline below is a framework for expectations, not a schedule that applies to everyone. The pace varies from person to person and depends on your starting weight, your diet, your physical activity and how you respond to the dosage. After month 6, weight loss in research usually continues, more slowly, until about a year after the start. After that, weight stabilises.
When do you see results on the scales?
Most people see a clear difference on the scales after 4 to 8 weeks. That coincides with the step from 2.5 mg to 5 mg, the first dosage at which the effect on appetite becomes noticeably stronger. People who see results earlier often mainly lose fluid at first.
Weigh yourself once a week, on the same day and at the same time. Daily weighing shows fluctuations in fluid and stomach content that say nothing about fat loss. A steady line over weeks is what counts, and that is also what the doctor looks at.
How much weight do you lose with Mounjaro?
How much weight you lose varies from person to person; what research shows is an average. In the SURMOUNT-1 trial, published in The New England Journal of Medicine, adults with obesity lost an average of 16 percent of their body weight in 72 weeks on 5 mg, 21.4 percent on 10 mg and 22.5 percent on 15 mg, compared with 2.4 percent on placebo. All participants also received support with diet and physical activity.
The European Medicines Agency (EMA) summarises it like this: more than 85 percent of participants using Mounjaro lost at least 5 percent of their body weight, compared with 35 percent in the placebo group. That also means a small group notices little effect.

Why the dosing schedule sets the pace
The dosing schedule determines how fast Mounjaro works for you, because the first 4 weeks on 2.5 mg are meant for getting used to it, not for losing weight. The schedule according to the Farmacotherapeutisch Kompas: start with 2.5 mg a week, after 4 weeks move to 5 mg, and then, if needed, increase in steps of 2.5 mg after at least 4 weeks each time, up to a maximum of 15 mg.
That build-up has one reason: limiting side effects. People who go up too quickly more often get nausea, vomiting and diarrhoea, and stop sooner as a result. A gradual build-up achieves more in the long term than a fast start.
Not everyone needs the highest dosage. Many people do well on 5 or 7.5 mg. The doctor only increases the dose if the effect falls short and the side effects allow it. Still hungry or little effect: is that normal?
Yes, hunger in the first weeks is normal. On 2.5 mg, appetite suppression is deliberately mild, and in some people the effect only really gets going on 5 or 7.5 mg. If you still notice little after 8 to 12 weeks on 5 mg, discuss it with your doctor; the next dose step is usually the solution.
Hunger also often has causes other than the dosage: too little protein, too little sleep, stress, or eating out of habit rather than appetite. Side effects in the first weeks: what is normal and when do you call the doctor?
Side effects usually occur in the first weeks and after each dose increase, and ease after that. According to apotheek.nl, the patient information website of the Dutch pharmacists' organisation KNMP, the most common are stomach and bowel complaints: nausea, diarrhoea, constipation and a full feeling. Tiredness and redness at the injection site have also been reported. The Farmacotherapeutisch Kompas describes these complaints as usually mild to moderate in severity.
Normal in the first weeks: mild nausea, especially after large or fatty meals, changed bowel habits and somewhat less energy. Smaller portions, eating slowly and drinking enough usually help.
Call your doctor if you have:
- Persistent severe abdominal pain, possibly radiating to the back: this can indicate inflammation of the pancreas.
- Vomiting or diarrhoea that lasts longer than a few days, with a risk of dehydration.
- Signs of low blood sugar (trembling, sweating, palpitations), especially if you also use other diabetes medication.
- An allergic reaction: swelling of the face, lips or throat, shortness of breath.

Can you lose 15 kilos in 3 months with Mounjaro?
For most people that is not realistic, and it is not the goal either. In the first 3 months you use a low to medium dosage and your body is still getting used to it. Doctors consider a pace of about half a kilo to one kilo per week responsible; faster increases the loss of muscle mass. How fast you lose weight varies from person to person.
Why am I not losing weight on Mounjaro?
The most common causes are a dosage that is still too low, an eating pattern that counteracts the effect, too little sleep or stress, and a plateau after rapid weight loss. Sometimes other medication or a thyroid problem plays a role. Discuss it with your doctor before you change anything yourself.
