Ozempic dosage and use: the complete schedule, step by step
Are you about to start Ozempic, or are you already using the pen and unsure about your schedule? Then there is one thing you want to know for certain: how much should I inject, and when? The short answer: you start with 0.25 mg per week and, under a doctor's supervision, build up step by step to a maximum of 2 mg per week.

What is the right Ozempic dosage?
The starting dose of Ozempic is 0.25 mg of semaglutide, once a week as an injection under the skin. After 4 weeks, the doctor increases the dose to 0.5 mg per week. After that, the dose can go up in steps of at least 4 weeks to 1 mg and a maximum of 2 mg per week.

The official Ozempic dosing schedule (0.25 to 2 mg)
The Ozempic dosing schedule follows a fixed build-up principle: each step lasts at least 4 weeks. This is set out in the official patient information leaflet approved by the College ter Beoordeling van Geneesmiddelen (CBG, the Dutch Medicines Evaluation Board).

The starting dose of 0.25 mg is not a maintenance dose. This first step is only intended to limit side effects such as nausea, not to give the maximum effect yet. So do not expect any big change in those first 4 weeks.
Why build up so slowly? The active substance semaglutide mimics the gut hormone GLP-1. That hormone slows your gastric emptying and suppresses your feeling of hunger. Your stomach and bowels need time to get used to that. If you increase too quickly, you are more likely to suffer from nausea, a full feeling or diarrhoea. More about how fast Ozempic works.
Important to know: not everyone ends up on 2 mg. Which maintenance dose suits you is decided by the doctor based on your result and your side effects. For one person 0.5 mg is enough, another needs 1 mg or more.
If you do not tolerate an increase well, that is not a failure and not a reason to stop. The doctor can keep you on a step for longer than 4 weeks, or temporarily go back to the previous dose until the complaints subside. The schedule above is the official route, but the pace is tailored to you.
Incidentally, the schedule applies to everyone who uses semaglutide by injection, whether it has been prescribed for type 2 diabetes or for weight loss. How to use Ozempic: the pen step by step
You inject Ozempic once a week, on a fixed day, just under the skin of your abdomen, thigh or upper arm. The prefilled pen delivers the right amount itself; you do not need to measure anything. This is how to use the pen:
- Check the pen. Make sure the liquid is clear and colourless and that the pen is not past its expiry date.
- Screw a new needle onto the pen. Use a new needle for every injection.
- With a new pen, do the flow check. Turn the dose selector to the flow check symbol and press until a drop appears at the tip of the needle. You only need to do this the first time you use a pen.
- Set your dose. Turn the dose selector until the dose counter shows exactly your dose.
- Inject. Insert the needle just under the skin of your abdomen, thigh or upper arm and press the button until the counter shows 0.
- Count slowly to 6. That way you can be sure the full dose has been delivered before you withdraw the needle.
- Remove the needle. Dispose of it in a sharps container and put the cap back on the pen.
Change injection site every week. This prevents skin irritation and hardened spots. If, for example, you always inject in your abdomen, keep at least a few centimetres away from the previous spot and from your navel. You can change the day if you need to; just make sure there are at least 3 days between two injections.
Nervous about injecting? Many users know that feeling from their first weeks. The needle is thinner than a millimetre and the injection takes a few seconds. After the first time, almost everyone says the same thing: that was not as bad as I expected. Two more tips that help:
- Take the pen out of the fridge 15 to 30 minutes before the injection. Cold liquid stings more than liquid at room temperature.
- Inject into relaxed skin. Sit down, breathe out calmly and press the pen onto the skin smoothly but without force.
Looking for "GLP-1 ampoule instructions"? Then be alert. In the Netherlands, Ozempic and other registered GLP-1 medicines are supplied as a prefilled pen, not as a separate ampoule or vial. Semaglutide in ampoules or small bottles almost always comes from illegal suppliers, with no control over content, strength or sterility. So there is a reason why no instructions for it exist: no Dutch doctor or pharmacy supplies it in that form.
How many clicks are 0.25, 0.5 and 1 mg?
With Ozempic you do not need to count clicks: you turn the dose selector until the dose counter shows your dose. The pen is designed so that you can only select the dose that pen is intended for. The window shows the dose in milligrams, and that number is what counts.
So where does the question about clicks come from? Lists circulate online in which clicks are counted to inject a lower or in-between dose with a stronger pen, for example 0.5 mg from a 1 mg pen. We, like doctors and pharmacists, strongly advise against this. There are three concrete reasons:
- Clicks are not an official unit of measurement. The manufacturer designed and tested the pen for the dose shown by the dose counter, not for in-between settings.
- The lists contradict each other. Different numbers of clicks per dose circulate for the same pen. As a user, you cannot check which one is correct.
- One counting error is immediately a dosing error. Too much semaglutide means days of nausea or worse; too little means a treatment that quietly does not work.

Always use the pen that matches your dose. Does your pen not match your doctor's schedule, or are you unsure about the setting? Then do not inject, but first contact your doctor or pharmacist. That one phone call prevents a double or half dose.
Do you hear clicks while turning? That is expected: the dose selector clicks at each setting. Those clicks are just a sound made by the mechanism, not a dosing tool. The only thing that counts is the number shown in the dose window at the moment you inject.
How many doses are in one Ozempic pen?
Each Ozempic pen contains 4 doses of the strength shown on the pen. So one pen is exactly enough for 4 weeks: one injection per week. A 0.5 mg pen, for example, delivers 4 injections of 0.5 mg.
After those 4 doses, some liquid may remain in the pen. This leftover liquid is not an extra dose, so do not try to get a fifth injection out of it. The pen is filled for exactly 4 full injections, plus the liquid for the flow check.
If you work it out for a whole programme, you use one pen per month in the build-up phase, switching to a pen with a higher strength at each step. Your pharmacy supplies the right pen each time, according to your doctor's prescription. That way the packaging automatically keeps pace with the build-up schedule: the 0.25 mg pen covers exactly your first 4 weeks, the 0.5 mg pen the 4 weeks after that.
When you collect or receive a new pen, always check the strength on the label. Especially around a dose increase, there are two strengths close together in your fridge. A fixed habit helps: throw away the old pen as soon as the new step begins.
Missed or incorrect Ozempic dose: what now?
If you miss a dose of Ozempic, inject it as soon as possible, as long as that is within 5 days of the planned time. Have more than 5 days already passed? Then skip the dose and inject the next one on your usual day. Never inject a double dose to make up for a missed injection.
An example makes it concrete. Suppose your usual injection day is Sunday and on Wednesday you notice that you forgot to inject. That is within 5 days: you inject straight away, and you do your next injection on Sunday as usual. If you only realise on Saturday, you skip the missed dose and inject your normal dose on Sunday.
Going on holiday, or has travel disrupted your schedule? The basic rule still applies: at least 3 days between two injections, and if in doubt, check first. Anyone who misses doses for several weeks in a row also cannot simply continue on the old dose; sometimes the schedule then needs to be built up again. Never decide this yourself, but discuss it with your doctor.

Storing Ozempic: in and out of the fridge
Store unopened Ozempic pens in the fridge between 2 and 8 degrees. After opening, a more flexible rule applies: after the first injection, the pen keeps for another 6 weeks, and it may also be kept out of the fridge, as long as it does not get warmer than 30 degrees. So storing Ozempic is simpler than many users think.

Four more practical storage rules from the patient information leaflet:
- Never freeze. Do not place the pen against the cooling element or the back wall of the fridge. Do not use a pen that has been frozen.
- Keep the cap on. Always store the pen with the cap on; semaglutide is sensitive to light.
- Needle off. Store the pen without a needle attached. This prevents leakage and a blocked needle.
- Travelling? Within those 6 weeks, the opened pen can simply go in your bag, even without a cool bag, as long as it is not in the sun or above 30 degrees. Flying? Take the pen in your hand luggage; in the hold it can freeze.
The most convenient place in the fridge is the door or a middle shelf, away from the back wall. If necessary, write the date of the first injection on the pen or in your diary; then you never have to wonder when the 6 weeks are up. A useful detail for anyone who prefers to keep the pen out of the fridge after opening: that is allowed, for example in a drawer in the bedroom, as long as it does not get warmer than 30 degrees there.
Was your pen opened more than 6 weeks ago, or has it been in the sun or frost? Then first check with your pharmacist before injecting again. If you are unsure whether the fridge at home or on the road was cold enough, the same advice applies: ask first, inject afterwards.
How fast does Ozempic work and when do you increase the dose?
Semaglutide is active in your body from the first injection, but you only notice the full effect of a dose step after about 4 weeks. The substance remains active for days and builds up further with each weekly injection, until it reaches a stable level. That is why you only inject once a week, and that is also why each step in the schedule lasts at least 4 weeks: only then do you know what a dose really does.
Many users notice in the first weeks that their feeling of hunger decreases and they feel full sooner. Food you normally could not keep your hands off is suddenly easier to leave. That is the GLP-1 effect: fewer cravings, slower gastric emptying and feeling full sooner.

If the result falls short on your current dose, the doctor assesses whether increasing is useful and responsible. Side effects play a major role in that decision. According to Apotheek.nl (the Dutch pharmacists' patient information website), 10 to 30% of users experience nausea, diarrhoea, constipation or abdominal pain, usually temporarily and often gone again within 4 weeks. Ozempic dosage for weight loss: only under medical supervision
There is no separate "weight loss dose" of Ozempic: for weight loss, the same build-up schedule from 0.25 to a maximum of 2 mg per week applies. So how many mg you need to lose weight cannot be said in advance. The doctor determines your maintenance dose based on your result, your side effects and your health.
So do not let other people's experiences unsettle you. The fact that someone in a Facebook group is on 2 mg and you are on 0.5 mg says nothing about who is doing "better". A lower dose with a good result and few complaints is, medically speaking, the ideal outcome.
To be clear: according to the European Medicines Agency (EMA), Ozempic is officially registered as a medicine for type 2 diabetes. A doctor can also prescribe semaglutide for overweight, but that decision is tailored to the individual. That is exactly why a prescription, a medical intake and regular check-ups are not a formality, but a condition for doing this safely.
So never buy Ozempic without a prescription from websites or social media. In Europe, the Inspectie Gezondheidszorg en Jeugd warned (IGJ, the Dutch Health and Youth Care Inspectorate) about counterfeit Ozempic pens in circulation some time ago. With a fake, you do not know what you are injecting, let alone at what dose. The only safe route to buy Ozempic is through a doctor and a registered pharmacy.
