How fast does Ozempic work? The short answer

Ozempic starts acting on your appetite within a few days of the first injection, but the effect builds up over weeks. The active substance semaglutide reaches a stable level in your blood after 4 to 5 weeks, and every dose increase starts a new build-up. You often notice less hunger in week 1 or 2; weight loss usually follows from week 4 to 8.

In the first week, you mainly notice that you feel full sooner and think about food less. The starting dose of 0.25 mg is deliberately low: it is meant to let your body get used to it, not to lose a lot of weight yet.

Some users feel mild nausea, a full feeling or tiredness straight after the first injection. Others notice nothing in the first few days. Both are common, because the level in your blood is still low in the first week.

How does Ozempic work in your body?

When do you start losing weight with Ozempic?

Most users see a clear drop on the scales from week 4 to 8, and the weight loss then continues for months. The pace is highest at the start and levels off over time.

What research shows: in the STEP 1 trial, published in The New England Journal of Medicine, participants with overweight or obesity lost an average of 14.9% of their body weight over 68 weeks on semaglutide 2.4 mg per week, compared with 2.4% on placebo. That is an average from controlled research with the higher Wegovy dose and with lifestyle support; the result varies from person to person, and with the Ozempic doses the effect is generally lower.

Why are you not losing weight on Ozempic?

If your weight stalls after 8 to 12 weeks, the cause is usually one of these five: you are still on a low dose, you are eating as much as before despite feeling less hungry, you are drinking calories (soft drinks, juice, alcohol), you are taking medication that causes weight gain, or there is an underlying cause such as an underactive thyroid.

Four factors determine the pace: your dose and dosing schedule, your eating pattern, other medication and conditions, and sleep and stress. The dose is the biggest factor: on 0.25 mg you notice less than on 1 mg, and that is exactly the point of the build-up.
