How buying Rybelsus through the legal route works

The legal route consists of four steps: an assessment by a doctor, a prescription, dispensing by a recognised pharmacy and support during use. In practice, it looks like this:

1. Assessment by a doctor. You discuss your weight, BMI, health, medication use and previous attempts to lose weight. The doctor assesses whether medical weight loss is right for you and whether a tablet is the right form in your situation.
2. Prescription. If it is suitable, the doctor writes a prescription with the starting strength and the dosing schedule.
3. Pharmacy. A recognised Dutch pharmacy checks the prescription and supplies the tablets, at the pharmacy or by home delivery.
4. Support. The doctor monitors how you respond, adjusts the strength and assesses whether you continue.

That may seem cumbersome compared with a webshop, but these very steps are what make it safe. Semaglutide has side effects and is not suitable for everyone; the assessment beforehand and the contact during use are not a formality.

Why online sellers without a prescription are unsafe

According to the Inspectie Gezondheidszorg en Jeugd (IGJ) (the Dutch Health and Youth Care Inspectorate), sellers who sell Rybelsus without a prescription are fake pharmacies: they look like a real pharmacy, but they are not. The inspectorate warns that such medicines can contain too much, too little or no active substance at all, and that an incorrect dose is dangerous and sometimes life-threatening.

How to recognise a legal provider:

• The provider asks for a prescription from a doctor or has a doctor assess you before the prescription.
• The website displays the EU logo for registered online pharmacies.
• The provider is listed on aanbiedersmedicijnen.nl, the government register of legal online providers.
• You never pay in advance to a foreign party without Dutch pharmacy details.

For whom does a doctor consider Rybelsus?

According to the medicines information database of the CBG (the Dutch Medicines Evaluation Board), Rybelsus is registered for adults with type 2 diabetes that is insufficiently controlled with diet and exercise alone. Use for weight loss without diabetes falls outside that registration. A doctor can then only prescribe it after their own consideration, as also happens with Ozempic. For medical weight loss, the usual thresholds apply: a BMI of 30 or higher, or from 27 with a weight-related condition such as high blood pressure, type 2 diabetes or sleep apnoea. The doctor also takes contraindications into account, such as pregnancy or wanting to conceive, a previous pancreatitis, severe kidney or liver problems and an eating disorder.

The doctor assesses whether a tablet suits your situation. The tablet requires discipline: take it every morning on an empty stomach with a sip of water and then wait 30 minutes before eating, drinking or taking other medicines. If you find that difficult, you are often better off with a weekly injection.

Which strengths of Rybelsus are there?

Rybelsus comes in two series of strengths: the original tablets of 3, 7 and 14 mg and the new formulation of 1.5, 4 and 9 mg. According to the College ter Beoordeling van Geneesmiddelen (the Dutch Medicines Evaluation Board), the old tablets are being replaced by the new ones, which are absorbed better: a lower dose gives the same effect.

Both series are temporarily available side by side and the colours of the packaging have stayed the same. So check with every delivery that the strength on the box matches your prescription. The 25 mg tablet you come across online is not Rybelsus but the new Wegovy tablet for weight management.

What does Rybelsus cost at the pharmacy?

At the pharmacy, Rybelsus costs about €3.30 per tablet, regardless of strength, so about €99 per 30 tablets. That follows from the cost overview in the Farmacotherapeutisch Kompas of Zorginstituut Nederland (the Dutch National Health Care Institute), accessed 22 September 2026, excluding the pharmacy's dispensing fees. Reimbursement is only available for type 2 diabetes under the conditions of the Dutch medicine reimbursement system. If you use Rybelsus to lose weight, you pay for the tablets yourself. Is Rybelsus the same as Ozempic?

Rybelsus and Ozempic contain the same active substance, semaglutide, but differ in form and dosage. Ozempic is a weekly injection of 0.25 to 2 mg; Rybelsus is a daily tablet of 1.5 to 9 mg (or 3 to 14 mg in the old formulation). The tablet needs a much higher dose because only a small part is absorbed through the stomach.

Both are registered for type 2 diabetes. For weight management, Wegovy is the registered semaglutide medication, since 2026 also as a tablet. Which medication suits you best from a medical point of view is a decision for your doctor.

Rybelsus or an injection: what suits you?

A tablet suits people who are afraid of needles and can manage a strict morning routine; a weekly injection suits people who value convenience and predictable absorption. The injection gives a more stable level in the blood; with the tablet, the level varies more from day to day, depending on how you take it.

Can I get semaglutide without a prescription?

No. All medications containing semaglutide (Rybelsus, Ozempic, Wegovy) are prescription medicines. Sellers who sell them without a prescription are acting illegally and regularly supply counterfeits.

Can I order medicines online without a prescription?

Only over-the-counter medicines. You can legally order prescription medicines online from a registered pharmacy, but always with a prescription from a doctor. Check the EU logo and the aanbiedersmedicijnen.nl register.

Can I go to the pharmacy for Rybelsus without a prescription?

No, the pharmacy only dispenses Rybelsus on prescription. You can, however, ask the pharmacist for information about the medication and the costs.

What is a replacement for Rybelsus?
