The 3, 7 and 14 mg strengths all cost €3.29 per tablet. So the strength does not determine what you pay, only how much semaglutide you take in per day.

You start on 3 mg; after 4 weeks the doctor usually increases this to 7 mg and, if needed, later to 14 mg. So the first month on 3 mg costs the same as a month on 14 mg.

The new Rybelsus tablets of 1.5, 4 and 9 mg: what changes in the price?

Rybelsus has recently also become available in a new tablet formulation with strengths of 1.5, 4 and 9 mg. These tablets are absorbed better in the stomach, so a lower strength gives the same effect as the old one. The price per tablet is the same as for the existing strengths.

Important: the new and the old tablets are not interchangeable one to one. A 4 mg tablet from the new range roughly corresponds to 7 mg from the old range. If you switch formulation, the doctor determines the right strength. Check with every new pack which range you have been given.

Is Rybelsus covered by health insurance?

Rybelsus is reimbursed from basic health insurance (basisverzekering) for type 2 diabetes, but only under conditions. According to the GVS advice from Zorginstituut Nederland from 2020 (the GVS is the Dutch medicines reimbursement system), reimbursement applies to people with type 2 diabetes and a BMI above 30 for whom metformin and a sulphonylurea do not work well enough, or as an addition to metformin and basal insulin.

If you use Rybelsus without diabetes, for example to lose weight, the insurer reimburses nothing. Supplementary insurance (aanvullende verzekering) generally does not cover it either. What does Rybelsus cost if you pay yourself, per month and per year?

If you pay yourself, Rybelsus costs about €99 per month and €1,200 per year for the medication, plus the pharmacy's dispensing fees for each supply. Then there are the doctor's costs: the consultation, the prescription and the support while you use it.

Rybelsus or Ozempic: the difference in price and use

Rybelsus and Ozempic contain the same substance, semaglutide, but differ in form, how you take them and price. Rybelsus is a daily tablet; Ozempic is a weekly injection.

Besides the price, ease of use counts. A tablet requires discipline: every morning on an empty stomach, with a sip of water, and then waiting 30 minutes before eating or drinking. An injection you take once a week.

In the Netherlands, Rybelsus is not registered as a weight loss medicine. The tablet is approved for type 2 diabetes; using it to lose weight without diabetes is off-label. That is only possible if a doctor considers it responsible in your situation and supervises its use.
