Reimbursement conditions for type 2 diabetes

For type 2 diabetes, basic health insurance only reimburses Mounjaro if you meet the GVS conditions. In short, these conditions come down to other medicines, such as metformin, not working well enough or not being suitable. Your GP (huisarts) or internal medicine specialist (internist) assesses this and writes the prescription. Read more about how to buy Mounjaro.

Since 1 May 2026, the conditions have been broadened on the advice of the Zorginstituut. People with type 2 diabetes and a very high risk of cardiovascular disease according to the NHG-Standaard (the guideline of the Dutch College of General Practitioners) can now also get Mounjaro reimbursed, in combination with metformin and/or an SGLT-2 inhibitor. Several older articles on other sites don't yet mention this extension.

Even with reimbursement, you usually pay something yourself. The deductible (eigen risico) of €385 per year always applies first. In addition, a personal contribution (eigen bijdrage) may apply per dose; by law this is capped at €250 per year for all your medicines together. For part of that personal contribution, the manufacturer has a refund scheme for the diabetes indication.

Why Mounjaro isn't reimbursed for weight loss

Mounjaro is approved in Europe for weight management, but in the Netherlands it has not yet been included in the reimbursement system for that use. Those are two different decisions. The European Medicines Agency (EMA) approved the active substance tirzepatide for adults with a BMI of 30 or higher, or from 27 with a weight-related condition. Whether a medicine is then also reimbursed is decided separately by the Dutch government, on the advice of the Zorginstituut.

As long as it hasn't been included in the GVS, no health insurer reimburses Mounjaro for weight loss. An example: if you have a BMI of 31 and no diabetes, you can only get the medicine on prescription and at your own expense. That feels unfair when losing weight on your own keeps failing, but it is the current reality.

Will obesity be reimbursed?

Perhaps, but not in the short term. The Zorginstituut is working on a GVS advice on tirzepatide for obesity and is assessing it in stages: first the groups with the heaviest health burden, such as people with a BMI of 30 or higher with additional conditions and people with a BMI of 35 or higher.

This is where the planning currently stands:

• March 2026: manufacturer's dossier complete, assessment started
• October 2026: first discussion in the Wetenschappelijke Adviesraad (WAR, the institute's scientific advisory board)
• After that: second WAR discussion, advice and eventually a decision by the minister; these dates are not yet known

So don't count on reimbursement for obesity arriving in 2026. If you don't want to wait for the politicians in The Hague, paying yourself with medical supervision is currently the only route.

What does Mounjaro cost if you pay yourself?

Without reimbursement, Mounjaro costs roughly €185 to €420 per four weeks at the pharmacy, depending on your dose. The indicative prices on medicijnkosten.nl, the Zorginstituut's official price database, range from about €46 per weekly pen for the 2.5 mg starting dose to about €419 per four-week pen for the highest dose of 15 mg. The pharmacy's service fees come on top of that.

Reimbursement per health insurer

Which insurer you're with makes no difference for Mounjaro. The reimbursement rules come from the national GVS and apply to the basic health insurance of all insurers. Zilveren Kruis, VGZ, CZ and Menzis therefore follow the same line: reimbursed for type 2 diabetes under the GVS conditions, not reimbursed for weight loss.

Not reimbursed? These are your options

No reimbursement doesn't mean you're on your own. These are your three realistic routes:

1. The combined lifestyle intervention (gecombineerde leefstijlinterventie, GLI): a reimbursed lifestyle programme through your GP, without medication. A good first step, although many people with a BMI above 30 have already tried such an approach without lasting results.
2. The manufacturer's refund scheme: only relevant if Mounjaro is reimbursed for your type 2 diabetes and you pay a personal contribution.
3. Paying yourself within a medical treatment: you only start after assessment by a doctor, with a prescription, check-ups and support with side effects.

If you choose that last route, do it safely. Never buy Mounjaro without a prescription through web shops or social media: that is illegal and the risk of counterfeits is high. The safe route always goes through a doctor who assesses whether the medicine suits you, and a registered Dutch pharmacy that supplies it.

Which insurance covers Mounjaro?

No health insurer reimburses Mounjaro if you use it to lose weight. For type 2 diabetes, all insurers reimburse the medicine through basic health insurance, under the same national GVS conditions. So there is no difference between Zilveren Kruis, VGZ, CZ and Menzis.
